Step-by-Step Guide to Accessing NJ4S-Essex Services

 

Here at NJ4S-Essex, we require different consent forms and action steps for our tier services to ensure that our hub stays compliant. This page serves as a central location to help school representatives collect the necessary consent forms and links for applications submitted through Salesforce. If you need assistance navigating this page or accessing our services, please reach out to our scheduling coordinators via email or by phone.

 

Here is a quick guide to ensure you’re fully registered and ready to access our services:

Once submitted, it may take 7-10 days for you to be entered into the system. Look out for an email with the subject: “Authorization for DCF Statewide Student Support Services NJ4S,” which will contain your Salesforce access instructions.

If you’ve already received the authorization email, simply visit the Home (nj.gov), sign in, and select “Start a New Application” to begin.

 

Here is a breakdown of our different tiers and what actions are required to access our services:

 

Tier 1 (Open to Everyone):

For parent or student presentations (workshops, assemblies), no application is needed. Just email [email protected] with your requested topic, time, and estimated attendance

 

Tier 2:

Criteria: Our Tier 2 services are offered to students and parents with students in 6th – 12th grade.

Action:

Application submission via Salesforce is required and submitted by the school representative.

School representatives must ensure consent forms are collected and completed by the parent or guardian Tier 2 Group NJ4S Participation Consent Form.

Online JotForm for Tier 2 consent forms in multiple languages:  English Tier 2 Group Form,  Spanish Tier 2 Group Form,  Portuguese Tier 2 Group Form  and Creole Tier 2 Group Form 

Student representative must complete the Students EBP List (jotform.com) for each group.

 

Tier 3:

Criteria: For students that require clinical services, often involving confidential or sensitive information. Our Tier 3 services are offered to students in 6th – 12th grade.

Action:

Application submission via Salesforce is required.

Notice Regarding Consent Forms and Student Information:

  • Consent Forms:
    Consent forms should not be emailed under any circumstances to our staff. Our clinicians and prevention consultants will come to collect all physical copies of consent forms. Also, all consent forms must be collected before services can begin.
  • Sharing Student Information:
    When sharing student names or other sensitive information with staff, ensure that emails are encrypted to maintain confidentiality and data security.
