Join the free Take Control of Your Health workshop, Family Connections’ Chronic Disease Self-Management Program (CDSMP), where you’ll get the support that you need and learn practical ways to deal with pain and fatigue, discover better nutrition and exercise choices, understand new treatment options, and learn better ways to communicate with your doctor and loved ones about your health.
Learn to manage chronic illnesses such as: Diabetes, cardiovascular disease, hypertension and/or arthritis through topics such as: Creating a health action plan, healthy eating, benefits of self-care and more.
Join us for a free 1 hour introduction class and then 2 hour CDSMP workshop, held each week for 6 weeks.

The Family Connections’ ADAPT team, in conjunction with the Essex County Division of Community Health Services and the Essex County Prosecutor’s Office, will be hosting the 6th Annual Essex County Interfaith Symposium. Join us to learn how spirituality and faith has an impact in reducing substance abuse and mental health issues during the pandemic.
